TERMS AND CONDITIONS OF ENROLLMENT



Personal Information and Photos



I understand that my personal information will not be released to any third party and is for the exclusive use of communication from Fourth Creek Musik.



If I have agreed to the photo release, I have given my permission to use photos or videos of myself or my child taken at Fourth Creek Musik classes or special events. I understand names will not be used without my permission.



If I have declined the photo release, I will speak to my educator about my preferences in person to discuss how best to support and respect my preferences.



Liability Release



With my enrollment, I trust that Fourth Creek Musik and its staff do everything possible to keep us safe. I release any and all rights and claims for damages against Gina Terry, Fourth Creek Musik, the staff of Fourth Creek Musik, and McCormack Realty in the unlikely event of illness or injury sustained by myself or my child)ren during the course of my enrollment.



Missed Classes



I understand that:

Should I miss class due to family illness or travel I have unlimited makeup classes that I can schedule through the my.kindermusik.com account so long as I am enrolled.

Classes cancelled due to inclement weather or teacher absence will not be rescheduled.

Free bonus events for enrolled families may be available periodically to replace missed or cancelled classes.

I must be fully enrolled (not paused) to participate in makeup classes and bonus events.





Enrollment Fees



I understand that:

Registration fees and monthly enrollment payments are non-refundable.

My enrollment fee is autodrafted to my credit card on file on the First (1st) of the month.

Payments that are declined, returned, reversed, disputed or otherwise remain unpaid are subject to a $25 fee. This does not replace responsibility for any tuition due.

Should I need to withdraw or request a pause in enrollment, I will submit my request by email or text to Gina at least 30 days before the next scheduled monthly enrollment fee. Requests received after that time will take effect the following billing cycle.

Any pause request is subject to approval and may require a $25 enrollment hold fee.



Siblings



I understand that infant siblings may attend class at no cost contingent that I am hands-free to participate with my enrolled child. An advanced request may be made for older siblings with cancelled school (not illness) to participate occasionally.
